Life Care Planning (Cost of Future Care Assessment)
A Life Care Plan consists of a comprehensive assessment of a client’s care needs. The purpose of a Life Care Plan is to ensure the provision of necessary and reasonable medical needs, services and equipment that will allow an individual to maintain the quality of life that one would have continued to lead but for the injuries sustained as a result of their losses. It is a dynamic process which includes a review of available medical information, interview and assessment with the client, family members or other relevant parties.
Next steps involve detailed costing analysis of the client’s future care needs which may comprise of the following areas: medications, therapies, equipment, care requirements, home maintenance needs, vocational needs, transportation needs and child care requirements. Our Occupational Therapists will travel to the client’s home to conduct these assessments and quantify their care needs in their own community or environment.
